I grant permission for this child to be photographed or interviewed for Holy Spirit Catholic Church’s publications, including, but not limited to the website and bulletin. I understand photographs or quotations may be printed for dissemination in Holy Spirit Publications and on the website. I release HSCC & The Archdiocese of Atlanta from any responsibility of liability for any claims arising from the publication or reproduction of any photographs or interviews.
